Boston — Form Health has begun prescribing weight-loss medications through the new Medicare GLP-1 Bridge Program, which expands access to certain GLP-1 drugs for eligible Medicare Part D beneficiaries with obesity.
The digital obesity and cardiometabolic care provider said it has long accepted Medicare and has the clinical infrastructure needed to help eligible patients obtain GLP-1 medications as part of comprehensive, physician-led obesity care.
Form also coordinates with pharmaceutical manufacturer programs, allowing eligible patients to receive prescribing, clinical management and other support through a single obesity care provider.

The Centers for Medicare & Medicaid Services demonstration program runs from July 1, 2026, through Dec. 31, 2027. It operates outside the standard Medicare Part D benefit and requires participating clinicians to determine eligibility, submit prior authorization requests through a centralized processor, prescribe treatment when appropriate and provide ongoing clinical management.
Form said its American Board of Obesity Medicine-certified physicians and multidisciplinary care teams combine medication, when appropriate, with nutrition guidance, physical activity and behavioral support rather than treating GLP-1 drugs as a standalone solution.
The company said it is available to help eligible Medicare beneficiaries navigate the new coverage pathway as demand for obesity treatment grows.
