Boston, Massachusetts — Commonwealth Care Alliance has appointed Pablo Hernandez, M.D., MHCM, FAAFP, as Vice President and Chief Medical Officer, bringing more than 20 years of experience in community health, academic medicine and managed care.
In the role, Hernandez will provide clinical leadership as CCA continues expanding its integrated care model for members across Massachusetts, including individuals who are eligible for both Medicare and Medicaid.
“Dr. Hernandez is a pivotal addition to CCA’s leadership team as we expand our clinical partnerships and drive forward the high-quality, whole-person care our members rely on,” said John Koehn, President of CCA.
Koehn said Hernandez’s experience working with providers, government stakeholders and advocacy organizations serving dual-eligible populations will support CCA’s efforts to address medical, behavioral and social needs.
Hernandez most recently served as Chief Medical Officer at Edward M. Kennedy Community Health Center, a multisite federally qualified health center in Worcester, Massachusetts. There, he oversaw clinical operations, quality, population health, compliance and provider leadership.
He previously served as Senior Medical Director at Boston Medical Center HealthNet Plan, now WellSense Health Plan, where he oversaw the organization’s Senior Care Options program.
A board-certified Family Medicine physician, Hernandez holds a Master of Science in Health Care Management from the Harvard School of Public Health and a Doctor of Medicine degree from Universidad Central de Venezuela. He completed his Family Medicine residency at the University of Connecticut.
Hernandez has also held academic appointments as Assistant Professor of Family Medicine and Community Health at the University of Massachusetts Medical School and Clinical Assistant Professor in the Department of Family Medicine at Boston University School of Medicine.
He has served on the boards of the YMCA of Central Massachusetts, the Regional Environmental Council and Community Care Cooperative, where he chairs the Quality Committee. He is also a member of Massachusetts’ Delivery System Technical Advisory Committee and the National Association of Community Health Centers’ Clinical Committee.
“Throughout my career, I’ve been privileged to care for patients with the greatest medical and social needs,” Hernandez said. “Joining CCA is a natural extension of that work and an opportunity to build on the organization’s longstanding commitment to serving older adults and individuals with complex health needs.”
Hernandez said he plans to work with CCA members, healthcare providers, community organizations and public-sector partners to improve access, health outcomes and person-centered care across Massachusetts.
